Recently, Seth fell into another Diabetic Coma or Code Blue! This time 
he fell so hard he broke a tooth and received a cut above his eye. He is 
currently receiving stabilizing sugars in the hospital, has a headache, 
is being monitored and is getting X-rays.

Seth has been suffering from uncontrolled diabetes for over 15 years 
now. His sugars go up to the 400’s, then down so low he falls into a 
diabetic coma. Each diabetic coma could end in death if not noticed and 
treated right away or can result in another fall.

We are asking that Seth be given an insulin pump/sugar monitor 
immediately to avert these near fatal incidents. He had a consultation 
with a endocrinologist in early October 2016 and was told the paperwork 
to receive a insulin pump would be completed immediately. On November 
20th Seth wrote a grievance (see attached) because he has not yet 
received the pump. Despite the endocrinologists recommendation, call in 
campaigns, and the grievance - months later and Seth still has no pump. 
The NYS DOCCS is completely aware of the situation and is stalling - any 
other incident of low sugar is dangerous and life threatening.

*Seth's constant extreme fluctuations in sugar levels have led to him no 
longer be able to tell when his sugars are high or low. *A sugar monitor 
sends a loud warning if his sugars are beginning to get either too high 
or too low and the pump allows him to use small doses of insulin when 
needed.

*1. Immediate provision of an Insulin Pump/Sugar Monitor to Robert Seth 
Hayes 74A2280. Seth needs this device immediately!*

*2. A Diabetic Diet that consists of fresh fruits and vegetables and all 
the current nutritional recommendations for diabetics - n**ot the 
problematic diabetic diet that is currently being issued. *
